Job Title: Community Case Management Program (CCMP)(Short-Term Hire)

Specific Duties andResponsibilities
CCMP Program Coordination and Implementation Support:

  • Support the CCMP Senior Associate in coordinating implementation of CCMP activities across Lagos, Kaduna, and Kogi States in line with national guidelines, program requirements, and organizational standards.
  • Maintain regular communication and follow-up with CCMP State Focal Persons to track implementation progress, planned activities, deliverables, emerging challenges, and agreed actions.
  • Support state teams to implement CCMP strategies aimed at improving case finding, enrollment, linkage to HIV care and treatment, retention, and access to integrated services for children and adolescents.
  • Support consolidation and documentation of CCMP achievements, planned outputs, outcomes, and implementation progress across the three states.
  • Identify implementation bottlenecks and promptly escalate issues requiring technical or management attention to the CCMP Senior Associate.

Mentoring, Supportive Supervision and Capacity Strengthening:

  • Support the CCMP Senior Associate and State Focal Persons in coordinating mentoring, coaching, supportive supervision, and on-the-job capacity strengthening for CBO staff and CCMP caseworkers.
  • Participate in selected supportive supervision and monitoring visits to assess implementation progress, identify gaps, and support follow-up of agreed actions.
  • Document findings, recommendations, and action points from supportive supervision and monitor their follow-up to completion.
  • Facilitate sharing of good practices and lessons learned across the three supported states.

Program Monitoring, Data Review and Reporting:

  • Coordinate the collation and documentation of monthly CCMP programmatic performance reports from CBOs and health facilities through the State Focal Persons.
  • Review submitted reports for completeness, consistency, accuracy, and timeliness, and follow up on identified gaps or discrepancies.
  • Support consolidation of state-level program performance information for routine CCMP reporting and management review.
  • Maintain updated trackers of key CCMP indicators, activities, targets, deliverables, and outstanding action points across Lagos, Kaduna, and Kogi States.
  • Support routine review and interpretation of program performance data to identify implementation gaps and areas requiring corrective action.
  • Support preparation of CCMP program updates, presentations, reports, and other documentation required by the program and organization.

Linkage and Integrated Service Delivery:

  • Support State Focal Persons to strengthen linkages and coordination between CBOs, health facilities, and relevant State/LGA structures for effective planning and delivery of CCMP services.
  • Support monitoring of referral and linkage mechanisms between community and facility platforms to promote continuity of care for children and adolescents.
  • Support implementation of strategies to improve access to integrated services for vulnerable adolescents and other beneficiaries.
  • In collaboration with the clinical team, support the integration and monitoring of screening for common opportunistic infections within the CCMP service package at health facility and community levels.
  • Support identification and follow-up of service delivery gaps affecting children and adolescents receiving CCMP services.

Quality Improvement and Program Performance:

  • Support implementation of quality improvement initiatives aimed at strengthening CCMP service delivery and improving program performance.
  • Support the identification of programmatic gaps and development of practical corrective actions in collaboration with State Focal Persons and the CCMP Senior Associate.
  • Track implementation of agreed corrective actions arising from data reviews, supportive supervision, program review meetings, and other monitoring activities.
  • Contribute to efforts to ensure achievement of established CCMP goals, objectives, planned outputs, and outcomes, and support appropriate documentation of results.

Documentation and Knowledge Management:

  • Support documentation of CCMP achievements, success stories, best practices, lessons learned, and implementation experiences across the supported states.
  • Maintain organized records of CCMP reports, supervision findings, meeting outputs, action trackers, and other relevant program documentation.
  • Support preparation of technical briefs, work plans, presentations, meeting reports, and other program documents assigned.
  • Participate in CCMP review meetings, technical sessions, stakeholder engagements, and other relevant program activities.
  • Perform other CCMP-related duties as assigned by the CCMP Senior Associate.

Performance Measures
Performance will be assessed based on the individual’s contribution to effective implementation and coordination of CCMP activities, including:

  • Timeliness and quality of collation, review, consolidation, and submission of CCMP programmatic reports from Lagos, Kaduna, and Kogi States.
  • Completeness and regular updating of CCMP performance, activity, deliverable, and action trackers.
  • Timely follow-up and documentation of implementation issues, action points, and agreed corrective measures across supported states.
  • Number and quality of mentoring, coaching, supportive supervision, monitoring, and quality improvement activities supported and documented.
  • Evidence of strengthened coordination and referral/linkage between CBOs, health facilities, and State/LGA structures.
  • Contribution to improved enrollment of eligible children and adolescents and linkage to health facilities for HIV care and treatment services.
  • Contribution to improved access to integrated services among vulnerable adolescents.
  • Contribution to improved delivery of counseling, case management, and other approved CCMP support services for eligible children and adolescents.
  • Quality and timeliness of documentation of CCMP achievements, best practices, success stories, lessons learned, and implementation outcomes.
  • Overall contribution to achievement and documentation of established CCMP program goals, targets, planned outputs, and outcomes.

Requirements

  • Bachelor’s Degree in Public Health, Nursing, Social Sciences, Community Health, or a related field.
  • Minimum of three (3) years’ relevant experience supporting implementation of community-based health, HIV, child health, adolescent health, or related programs.
  • Experience working with CBOs, health facilities, state/LGA structures, and community-based service delivery programs.
  • Experience in program coordination, monitoring, data review, reporting, and documentation.
  • Working knowledge of HIV programming, community case management, and integrated service delivery approaches for children and adolescents is desirable.
  • Experience supporting programs across multiple geographic locations will be an advantage.

Abilities:

  • Demonstrated ability to coordinate and follow up on multiple activities and stakeholders across different locations.
  • Ability to work collaboratively with state focal persons and implementing partners while maintaining effective communication and accountability.
  • Ability to review program information, identify inconsistencies or implementation gaps, and support appropriate follow-up.
  • Strong organizational skills with the ability to manage competing priorities and meet deadlines.
  • Ability to work independently while maintaining appropriate communication and escalation with the supervisor.
  • Demonstrated ability to support mentoring, coaching, and collaborative problem-solving.

Skills:

  • Excellent written and verbal communication skills.
