The Social Worker/Case Manager at the Children Against Torture and Crime Foundation will be responsible for providing comprehensive support services to children and families affected by torture and crime. This role involves case management, advocacy, and collaboration with other service providers to ensure the well-being and safety of the children served by the foundation.
Key Responsibilities:
Case Management:
- Conduct intake assessments to determine the needs of children and families.
- Develop individualized care plans and coordinate services to address identified needs.
- Monitor and evaluate the progress of each case, making adjustments as necessary.
Advocacy:
- Advocate for the rights and best interests of children affected by torture and crime.
- Collaborate with legal, medical, and educational professionals to ensure comprehensive care and support.
- Participate in community outreach and public awareness efforts to promote the foundation\’s mission.
Direct Support:
- Provide direct support and counseling to children and families, including crisis intervention, emotional support, and guidance.
- Facilitate support groups and educational workshops for children and caregivers.
- Connect clients with appropriate resources, such as housing, healthcare, and educational services.
Collaboration & Coordination:
- Work closely with other service providers, including mental health professionals, social workers, and community organizations, to ensure a holistic approach to care.
- Coordinate and participate in case conferences and team meetings to review and discuss cases.
- Maintain strong relationships with stakeholders to enhance service delivery and support.
Documentation & Reporting:
- Maintain accurate and up-to-date case files, ensuring confidentiality and compliance with legal and ethical standards.
- Prepare and submit regular reports on case activities, outcomes, and challenges.
- Collect and analyze data to inform program decisions and improvements.
Professional Development:
- Stay informed about best practices and developments in social work, case management, and child welfare.
- Participate in training and professional development opportunities to enhance skills and knowledge.
- Contribute to the foundation\’s learning and improvement by sharing insights and experiences.
Qualifications:
- Bachelor\’s degree in Social Work, Psychology, or a related field
- Minimum of 2 years of experience in social work or case management, preferably with a focus on children and families.
- Strong understanding of issues related to child welfare, trauma, and human rights.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Demonstrated ability to advocate for and support vulnerable populations.
- Proficiency in maintaining accurate records and documentation.
- Passion for the mission and values of the Children Against Torture and Crime Foundation.
